1915(i) Care Coordination


The 1915(i) Medicaid program allows individuals with behavioral health conditions to access enhanced home and community-based services. A Care Coordinator plays a crucial role in this process by prioritizing the individual's needs, preferences, and voice through person-centered planning. This includes conducting thorough assessments, developing and monitoring care plans, creating crisis strategies, and coordinating necessary referrals and support. Eligible participants can work with their Care Coordinator to tailor a Plan of Care that may encompass a variety of services such as training for caregivers, transitional services, and support for housing, education, and employment.
